               Medical transcription is the process of transcribing voice recordings or notes that healthcare providers have dictated into medical records. In order to create written medical reports, such as patient histories, physical examination results, laboratory test results, operation reports, referral letters, etc., the medical transcriptionist must convert the dictation into written medical reports.

Assigning codes to diagnoses and procedures based on medical records and other health information is just one of a medical coder's responsibilities. Others include checking medical records for accuracy and completeness, ensuring that all codes are applied correctly, and making sure that codes are applied in accordance with established rules and regulations.
